recurring pinching pain in instep

    Hello Doctor,

    Since yesterday, I have had an odd pinching pain in my instep. It starts with a tingle, then an instant later, a pinching pain, then it subsides rather quickly (within a few seconds) only to be followed some 10 to 20 seconds later by another episode. It started yesterday and still continues this morning. It happens in one particular spot in the instep just about an inch or so above my right-foot's arch. The pain is not very great, but at night, with it repeating so quickly, it was very hard to sleep.

    I'm in my late 40s, male, and have neuropathy in my feet, but after repeated tests for diabetes (I'm overweight and my father had diabetes), all tests have come up negative for that condition. I am used to tingles in the toes and such, but this is new, and I am surprised by its repeated nature and the fact that it is always in the same spot.

    I'm an American living in Japan, so having some insight given in my mother tongue would help me communicate with my Japanese doctors.

    Any help would be appreciated.

  2. It sounds like the start of a nerve entrapment, perhaps tarsal tunnel syndrome, and/or early -- very early -- start of neuropathy. I hope this helps.
